Registered Nurse primary duties:
• Oversees the proper administration of total care to our residents.
• Provides general nursing care to residents in the Skilled Nursing Facility in compliance with state and federal regulations.
• Oversees work of Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), and Medication Aide (when applicable) to validate their compliance with nursing standards, state and federal regulations and laws, and Center policies.
• Works collaboratively with other members of the interdisciplinary healthcare team.
• Maintains all applicable standards of professional nursing practice.

Registered Nurse qualifications:
• Graduate of an accredited school of nursing with a valid state license and CPR certification.
• Demonstrated knowledge of nursing process, particularly as it relates to geriatric residents, long-term care, and rehabilitation.
• Strong clinical skills and ability to react appropriately in emergency situations.
• Impeccable verbal and written communication skills.
• Both experienced and new grad Registered Nurses encouraged to apply.

Registered Nurse primary duties:

• Oversees work of Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), and Medication Aide (when applicable) to validate their compliance with nursing standards, state and federal regulations and laws, and Center policies.
• Works collaboratively with other members of the interdisciplinary healthcare team.

Registered Nurse qualifications:
• Graduate of an accredited school of nursing with a valid state license and CPR certification.

We have a new employment opportunity for an MDS Coordinator in Chicago, Illinois. The chosen MDS Coordinator / RNAC will ensure accurate completion of the Resident Assessment Instrument (RAI) including the Minimum Data Set (MDS) as prescribed by State and Federal regulations.

This position reports to the DON and has supervisory responsibility with regard to the completion of the MDS / care planning process. The MDS Coordinator delegates assignments to the interdisciplinary team and monitors their progress to ensure compliance with the MDS process. As leader of the Care Plan team, this position has overall responsibility for coordination of resident care between departments.

The chosen candidate will be a Nurse with a current license in good standing and current CPR certification. A minimum of 2 years’ experience within Long Term Care and a minimum of 1 year of MDS 3.0 experience required. One year of supervisory experience strongly preferred.

RNs preferred, LPNs may be considered.

The MDS coordinator may need to be available on weekends and holidays on occasion. The chosen candidate must be available to work beyond normal working hours and in other related positions on a temporary basis.
