Location: McAlester OK (Southeastern Oklahoma)

Manages Case Management screens for all patients for discharge planning needs. Oversees development of the plan of care with the case managers for all patients and monitors progress. Most of the time will be spent coordinating the high-risk discharges and special needs of complex patients working with the multidisciplinary team (primary nurses, physicians, post-acute care manager). Will guide the care management of high-risk patients for the specific physician(s) he or she is assigned. Will manage the variances from the established plan of care. Cross-trained to perform utilization review as needed and when assigned to do the job. Must be detail-oriented and organized, with demonstrated computer and critical thinking skills. Must have effective communication skills as demonstrated with both internal and external customers.
Minimum Education
Graduate of an accredited school of nursing
Minimum Work Experience
3-5 years of combined clinical and management experience preferably with diverse clinical experience. Must possess an understanding of post-acute and ambulatory services, as well as all hospital departments. Must have excellent communication skills, ability to multitask, be proficient in directing other staff to carry out necessary functions in patient care delivery. The Manager of Case Management will be able to perform a complete patient assessment and participate in basic ADLs when required to facilitate patient movement throughout the continuum
Required Licenses/Certifications
Registered Nursing License
Current NIHSS certification
Required Skills, Knowledge, and Abilities
Strong organizational skills
Basic computer skills---familiar with key board, mouse, function keys
Knowledge of Microsoft Word and Excel or equivalent
Excellent people skills
Excellent oral and written communication skills
