Exciting opportunity exists for the right individual to work in our Coquitlam Head Office as a Supported Independent Living (SIL) Case Manager. The successful candidate will be joining a progressive non-profit organization with growing opportunities. We work with Adults with various disabilities to provide them with resources to live healthy and fulfilling lives at home and in the community.
Shifts are Monday to Thursday 10am to 3pm (Temporary one year MAT Leave with opportunities to become permanent). Hours may increase in time.
*Must be willing to travel throughout Lower Mainland and Fraser Valley
*Occasional evening and/or weekend work
*Position includes rotating on-call duties
Job Qualifications Include:
-Minimum Bachelor of Social Work or other Social Science
-Minimum of One (1) year of case management experience
-Minimum of One (5) year experience with individuals with disabilities
Job Requirements Include:
-Criminal record check
-TB test
-First Aid and CPR
-Physician's note
-Minimum of three (3) references
-Valid BCDL and Reliable vehicle to use for work purposes
Additional Requirements:
• Experience in the public or non-profit sector
• Experience working with individuals with physical, developmental disabilities, brain injuries, FASD, Autism, harm reduction, challenging behaviours and/or complex health issues, senior care
• Background in providing behavioural support strategies in an adult model is an asset
• Experience in the social services field
• Experience working in a team environment with little supervision
• Demonstrated ability to deal with crisis and emergency situations
• Ability to manage and coach a team of up to 20 support staff
• Computer literacy and experience with databases
• Excellent written and verbal communication skills
• Excellent time management skills
• Knowledge of community resources
• Ability to work within a multi-disciplinary team environment
• Adaptability and flexibility within the work environment
• Ability to encourage, motivate and involve client in community activities.
• Ability to communicate effectively (both verbally and in writing) with youth and their families, educational personnel, social workers and resource workers, service providers and partners
• Ability to consult, collaborate with, and support staff, volunteers and practicum students to assist in service delivery and ensure that interventions are integrated and coordinated to provide continuity of care.
• Clear understanding of the empowerment model as applied to supporting adults in their environment
Start Date: As soon as possible
Compensation starting at $23.47/hr and full benefits upon meeting eligibility requirements after three months.
