The Licensed Practical Nurse provides appropriate nursing care in the home environment of patients under the direction of the medical director or lead nurse practitioner. The LPN is responsible for inspecting a patient's home environment for possible hazards and dangers, providing specific patient care duties, and educating the patient and any caregivers living in the home about the patient's condition and needs. Findings by the house call LPN are to be communicated to the medical director or lead nurse practitioner to receive the appropriate treatment plan as ordered within the scope of practice for the LPN and outlined in the standardized procedures.
Additional responsibilities may include but are not limited to:
Completes an initial assessment of patient to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).Regularly re-evaluates patient nursing needsInitiates the plan of care and makes necessary revisions as patient status and needs change.Performs venipuncture, urine dipstick, blood glucose testing, PT/INR, HgB, Wound care, foley catheter care and insertion, enteral tube feeding and care.Facilitates patient transfers to alternate level of care.
Qualifications
Requires an LPN; 2 years of experience as an LPN; or any combination of education and experience, which would provide an equivalent background.Current LPN license in the applicable state required.Excellent written and verbal communication skillsBi-lingual (English/Spanish) preferred.Must be able to work independently.Must be able to travel to facilities and homes up to 40-80 miles per day.Basic computer skills and previous experience documenting in an EMR preferred.
