SUMMARY:
Management Business Solutions (MBS) is seeking a Clinical Pharmacist to work in Grand Rapids, MI. You will become the dedicated clinical pharmacist to support the transplant program for recipients and living-donors.
RESPONSIBILITIES:•Clinical pharmacy services will be provided to the transplant recipients during the inpatient, discharge and post-phases of transplantation and donation
•The pharmacy service will include identifying, solving and preventing medication related problems in the transplant population and living donors for the organ program.
•The pharmacist will provide education regarding the safe and effective use of medications in the post-transplant population to the healthcare team, patients and families.
•The transplant pharmacist will maintain documentation related to transplant pharmacy activities that are complaint with CMS regulations.
•This position will also maintain competency in more broad clinical pharmacy areas in order to provide support to the acute care clinical pharmacy services team.
QUALIFICATIONS:•Doctor of Pharmacy degree from ACPE-accredited institution
•Board Certified or Board-eligible in Pharmacotherapy
•2-5 years’ experience as a clinical pharmacist
•Completed a Pharmacy Practice (PGY-1) Residency
•Board Certified or Board-eligible in Pharmacotherapy
•Licensed pharmacist in Michigan (or eligibility to be licensed in Michigan)
•BLS/ACLS certification (or completed upon hire)
PREFERRED QUALIFICATIONS:•Transplant center experience, preferred
•Completed a Specialty (PGY-2) Residency in Solid Organ Transplant OR 3 or more years' experience in solid organ transplant including kidney transplantation for those without a PGY-2 Residency.
