CLINICAL DOCUMENTATION SPECIALIST SAN RAMON, CA REGION
Provide documentation and utilization of acute care services. Includes facilitation of appropriate physician documentation of care to accurately reflect patient severity of illness and risk of mortality. Will play a significant role in obtaining accurate and compliant reimbursement for acute care services and in reporting quality of care outcomes.
Minimum Experience/Skills:
Five (3) years clinical Acute Care or IP coding experience. ICD-9/CPT coding experience in an acute care setting as well as three (3) years Clinical Documentation Improvement experience required. Candidate must have knowledge of care delivery documentation system and related medical record documents. Candidate must have knowledge of age specific patient needs; must possess a strong broad-based clinical knowledge that includes patho-physiology and disease processes. Candidate must be capable of excellent written and verbal communication, critical thinking and interpersonal skills that build effective partnering relationships with physicians, nursing staff, hospital management staff, and HIM coding staff. Candidate must be able to work independently in a time-sensitive environment exhibiting strong computer literacy and familiarity with the operation of basic office equipment. Candidate must demonstrate assertive personality traits that will benefit ongoing physician communication. Candidate must have strong knowledge of the Medicare reimbursement system and coding structures/national coding guidelines.
Physical Demands:
This job requires the ability to stand and walk for long periods of time. A substantial part of your daily shift will be spent walking to and from patient care units. The job also requires that you stand for extended periods of time to do concurrent review of the patient record in the patient care units.
